Purpose: The TennCare CHOICES in Long-Term Care program allows those who are eligible for nursing home care to receive services at home or in a community setting.
Eligibility: Live in Tennessee, be 65 and older or an adult 21 or older with a physical disability, qualify for Medicaid through DHS, need the level of care received in a nursing home, have a safe home environment to receive services and have your needs met there, and the total cost of that care would not be more than nursing home care.
Top 3 Goals:
1. Provide a community-based cost effective alternative to institutional care
2. Help consumers remain at home for as long as safely possible
3. Help consumers maintain independence and dignity in their lives
Explanation of Services:
Available CHOICES services include Personal Care Visits, Attendant Care, Homemaker Services, Home-Delivered Meals, Personal Emergency Response System, Adult Day Care, In-Home Respite Care, In-Patient Respite Care, Assistive Technology, Minor Home Modifications, Pest Control, and Community-Based Residential Alternatives. Some of these services can be provided through Consumer Direction. CHOICES staff will complete a telephone assessment with the potential consumer or their representative. The Toll Free Number is 1-866/836-6678 to make contact with the CHOICES staff responsible for the telephone intake. Using the telephone assessment, the case management staff will schedule an in-home visit and complete an assessment to determine eligibility and prepare a service plan.
Number of Persons Served: In our sixteen counties, approximately 1000 individuals are served each year.
Source of Funding: Both Federal and State funds are utilized to provide services.
Counties Served: Anderson, Blount, Campbell, Claiborne, Cocke, Grainger, Hamblen, Jefferson, Knox, Loudon, Morgan, Monroe, Roane, Scott, Sevier and Union.
