My Support Connection
Purpose: My Support Connection provides connection to quality and affordable services for individuals and/or family caregivers.
Eligibility: No eligibility criteria. My Support Connection is for anyone seeking private pay services.
Top 3 Goals:
1. To assist consumers with making decisions about how best to maintain independence.
2. To provide a thorough home assessment of consumer’s needs.
3. To offer quality and affordable services when and how they are needed.
Explanation of Services: My Support Connection assists older adults, persons with disabilities and family caregivers with accessing quality and affordable services to help maintain independence for as long as possible. My Support Connection partners with local licensed and insured providers to conduct home visits and deliver services. My Support Connection offers ongoing quality assurance of services and assists consumers with concerns that are not resolved by the partnering provider. Services through My Support Connection are offered at low rates to keep care costs at a minimum. Costs depend on the hours and type of services selected. Consumers or their representative can make contact with My Support Connection staff by calling the toll free number (1-866-836-6678). Staff will complete a telephone screening with the potential consumer or their representative. My Support Connection will then contact the consumer to schedule a home visit to discuss the needs and connect the individual/family with services to meet those needs.
Source of Funding:Services are paid for by consumer.
