Options for Community Living
Eligibility
Individuals 18 years of age or older with functional limitations. These limitations are referred to as limitations with activities of daily living, such as bathing, dressing, eating, mobility, communications, and other problems that would limit an individual’s ability to live at home safely.

- To provide quality support services to 350 individuals each fiscal year.
- To provide improved information and referral services to individuals served through Options.
- To develop a strong service provider network in the region to assure we have multiple providers for each service in each county.
Explanation of Services
The services that are available through Options include Information, Referral, Case Management, Home Delivered Meals, Homemaker, and Personal Care. The options staff will complete a telephone assessment with the potential consumer or their representative. The toll free number is 1-866-836-6678 to make contact with the Options staff responsible for the telephone intake. Using the telephone assessment, the case management staff will schedule an in-home visit and complete an assessment to determine eligibility and prepare a service plan.

In the (16) counties, approximately 350 individuals are served each year.

State funds are made available through the Tennessee Commission on Aging and Disability.

Anderson, Blount, Campbell, Claiborne, Cocke, Grainger, Hamblen, Jefferson, Knox, Loudon, Morgan, Monroe, Roane, Scott, Sevier and Union.
